Basic Package

Essential support with light nursing oversight

What’s Included:

• Registered Nurse (RN): One initial assessment visit for care planning
• Licensed Practical Nurse (LPN): One visit every 15 days
• Home Support Worker: Daily in-home support for personal care, mobility, meals, companionship, and safety checks for 24*7
• Care Coordination: Appointment escort, medication reminders, and family updates

* This plan does not include ongoing RN visits, professional therapies, or specialty services. It is designed for patients who require basic support and limited nursing involvement.

Standard Package

Consistent nursing oversight with daily home support

What’s Included:

• Registered Nurse (RN): Two visits per month (every 2 weeks)
• Licensed Practical Nurse (LPN): One visit per week
• Home Support Worker: Daily in-home support for personal care, mobility, meals, companionship, and safety checks for 24*7
• Care Coordination: Appointment scheduling, medication reconciliation, and communication with healthcare providers
• Clients enjoy a higher level of comfort and convenience with fully  linen services included. All personal clothing e.g bed linens, and towels are regularly washed, changed, and neatly arranged to maintain a fresh, hygienic, and comfortable living environment.

* This plan does not include therapy services (OT, physiotherapy, dietitian, etc.) or specialty visits. These can be arranged separately if requested.

Premium Package

Comprehensive care with full professional and support services included

What’s Included:

• LPN Visits: Every 3rd day – included
• Licensed Practical Nurse (LPN): One visit every 15 days
• Home Support Worker: 24/7 in-home care (round-the-clock assistance)
• Social Worker (Easewell): One visit per month – included (provided by Easewell, not Eastern Health)
• Foot Care Nurse: One visit every 2.5 months – included
• Dietitian:  One visit per three month   – included (if extra consultations billed separately)
• Occupational Therapist (OT):  One visit per three month   – included (if extra visits billed separately)
• Physiotherapy: Available on demand via trusted third-party partners (fees billed separately)
• Recreation Therapy: One session per two week – included (extra sessions billed separately)
• Barber/Hairdressing: Every 15 days – included
• Dental Hygienist: first monthly visit for oral health check and hygiene care – included (any treatments during the visit billed separately)
• Transportation: One trip per month – included (via third-party provider(go bus, on demand ect); extra trips billed separately)
• Full Care Coordination: Direct communication with your healthcare providers, medication management, monitoring of chronic conditions, and regular family updates
